What is Social Prescribing?

Social prescribing is a service for people who are lonely and socially isolated, and want to improve their quality of life but need help connecting to local community groups, activities, and services.

Who is Social Prescribing for?

If you need a little support to meet new people, join local groups or activities, get more active, or improve your physical and mental wellbeing this may be the service for you.

How it works

Step 1: Contact Social Prescribing

You can contact the Social Prescribing Coordinator yourself, or you can be referred by your GP or any healthcare professional.

Step 2: Meet up

The Coordinator will contact you to see if you want to take part, and then arrange to meet for an informal, relaxed chat in a community venue in either Tuam, Athenry or Loughrea.

Step 3: Information

We will explore your interests and suggest local activities, groups, volunteer opportunities, or courses that are available in the area.

We can also direct you to support services based on your needs.

Step 4: Choose supports

We will help you develop a plan and support you to achieve your goal.

Step 5: Follow up

We will contact you to see how you are doing once you have joined a group/activity and will ask you for feedback on the service.

• Volunteer opportunities

• Support groups

• Information services

• Card games

• Sports clubs

• Running/walking groups

• Writing groups
